I, the undersigned participant, willingly engage in TruForm Pilates workouts, exercise programs, and related services. I understand that these activities, including yoga, Pilates, reformer Pilates, and various fitness routines, are recreational sports involving physical exertion.

By participating, I affirm that I am in good physical condition and assume any associated risks. I hereby release Fit Formula Pty Ltd (T/A TruForm Pilates) and their employees (collectively, the "Released Parties") from any claims or causes of action arising from my voluntary participation in TruForm Pilates workouts.

I acknowledge the potential for injury and absolve the Released Parties of liability for any present or future health conditions resulting from my involvement, including but not limited to muscle strains, broken bones, or other physical discomforts. I confirm that I have read and comprehended the above statements and agree to participate in TruForm Pilates at my own risk.
