Today, Between: FS8 Severna Park and Participant Named Below.
Contract Start Date: Start of membership as noted.
1.0 FEES AND PAYMENT
Contract Duration and Automatic Renewal: This contract is in force as of the date of submission, with automatic renewal for subsequent months, unless canceled by either party or with notification of 30 days in advance of the next billing date.
Agreement to Pay Recurring Fees:
By clicking the box below you agree that you have read and agree to the following:
(a) I agree to pay monthly recurring fees of my chosen membership as follows via Client authorized automatic credit card or checking account ACH charges on a monthly basis until I notify FS8 Severna Park in writing that I would like to terminate. I may cancel this Agreement without penalty or obligation within three (3) business days after the date this Agreement is signed. After the initial three (3) business day cancellation period, I may cancel this agreement at any time providing written notice at least thirty (30) days prior to my next billing date. I will be responsible for any payments due during the notice period.
(b) I hereby certify that I am the holder of the credit card, or an authorized signer on the bank checking account detailed below.
(c) I understand that I will be notified if my credit card or checking account payment fails to authorize for any reason, and that a $20 late fee will apply if I do not provide a valid credit card or checking account ACH information within 10 calendar days of the original rejection date.
(d) I understand that my service will be deactivated if my account becomes more than 90 calendar days late.
(e) I understand I will automatically be charged any bank fee imposed on the studio, plus a processing fee of up to $20, for any returned payment item due to closed accounts, insufficient funds etc.
(f) I understand that this membership automatically renews on a recurring basis and that my payment method will be charged accordingly until I cancel in accordance with the agreement terms.
(g) To the extent permitted by law, Client may be responsible for reasonable costs associated with the collection of any unpaid balances under this Agreement.
